The spectacular development of medical knowledge over the last two centuries has brought intrusive advances in the capabilities of medical technology. These advances have been remarkable over the last century, but especially over the last few decades, culminating in such high technology interventions as heart transplants and renal dialysis. These increases in medical powers have attracted societal interest in acquiring more such knowledge. They have also spawned concerns regarding the use of human subjects in research and regarding the byproducts of (...) basic research as in the recent recombinant DNA debate. As a consequence of the development of new biomedical knowledge, physicians and biomedical scientists have been placed in positions of new power and responsibility. The emergence of this group of powerful and knowledgeable experts has occasioned debates regarding the accountability of physicians and biomedical scientists. But beyond that, the very investment of resources in the acquisition of new knowledge has been questioned. Societies must decide whether finite resources would not be better invested at this juncture, or in general, in the alleviation of the problems of hunger or in raising general health standards through interventions which are less dependent on the intensive use of high technology. To put issues in this fashion touches on philosophical notions concerning the claims of distributive justice and the ownership of biomedical knowledge. (shrink)

This passage shows the perennial nature (...) of the problems of treating the patient as a person. It shows as well the historical'depth of philosophical interest in medicine. The history of philosophy includes more reflections upon medical ethics than the casual reader might suspect. Many of these reflections are pertinent to contemporary issues such as abortion and population control. Plato, for example, recommends abortion in cases of incest (Republic 5. 461c); and Aristotle argues for letting seriously deformed children die, while forbidding infanticide as a means of popUlation control, suggesting instead the use of early abortions. 'As to the exposure in rearing of children, let there be a law that no deformed child shall live, but that on the ground of an excess in the number of children... let abortion be procured before sense and life have begun; what mayor may not be lawfully done in these cases depends on the question of life and sensation' (Politics VII, 16,335 b20-26, [4]). Copyright © 2015 by the Society of Criti. Objective: Global lung stress varies considerably with low tidal volume ventilation for acute respiratory distress syndrome. High stress despite low tidal volumes may worsen lung injury and increase risk of death. No widely available parameter exists to assess global lung stress. We aimed to determine whether the volume delivered during a recruitment maneuver is inversely associated with lung stress and mortality in acute respiratory distress syndrome. Design: Substudy of an acute respiratory distress (...) syndrome clinical trial on esophageal pressure-guided positive end-expiratory pressure titration. Setting: U.S. academic medical center. Patients: Forty-two patients with acute respiratory distress syndrome in whom airflow, airway pressure, and esophageal pressure were recorded during the recruitment maneuver. Interventions: A single recruitment maneuver was performed before initiating protocol-directed ventilator management. Recruitment maneuvers consisted of a 30-second breath hold at 40 cm H2O airway pressure under heavy sedation or paralysis. VRM was calculated by integrating the flow-time waveform during the maneuver. End-inspiratory stress was defined as the transpulmonary pressure during end-inspiratory pause of a tidal breath and tidal stress as the transpulmonary pressure difference between end-inspiratory and end-expiratory pauses. Measurements and Main Results: VRM ranged between 7.4 and 34.7 mL/kg predicted body weight. Lower VRM predicted high end-inspiratory and tidal lung stress. After adjusting for Pao2/Fio2 and either driving pressure, tidal volume, or plateau pressure and positive end-expiratory pressure, VRM remained independently associated with both end-inspiratory and tidal stress. In unadjusted analysis, low VRM predicted increased risk of death. VRM remained significantly associated with mortality after adjusting for study arm. Conclusions: Low VRM independently predicts high lung stress and may predict risk of death in patients with acute respiratory distress syndrome. (shrink)

The US neutrino community gathered at the Workshop on the Intermediate Neutrino Program at Brookhaven National Laboratory February 4-6, 2015 to explore opportunities in neutrino physics over the next five to ten years. Scientists from particle, astroparticle and nuclear physics participated in the workshop. The workshop examined promising opportunities for neutrino physics in the intermediate term, including possible new small to mid-scale experiments, US contributions to large experiments, upgrades to existing experiments, R&D plans and theory. The workshop was organized into (...) two sets of parallel working group sessions, divided by physics topics and technology. Physics working groups covered topics on Sterile Neutrinos, Neutrino Mixing, Neutrino Interactions, Neutrino Properties and Astrophysical Neutrinos. Technology sessions were organized into Theory, Short-Baseline Accelerator Neutrinos, Reactor Neutrinos, Detector R&D and Source, Cyclotron and Meson Decay at Rest sessions.This report summarizes discussion and conclusions from the workshop. The idea of God’s action in history was prominent in the Biblical theology of the past generation. This theology was generally opposed to philosophic explication of its doctrine of God, and consequently it is difficult to say just what it meant by divine action. In any case, that movement faded and, with it, talk of God’s acts.

Whitehead does not provide us with a systematic account of the various types of experience to which the word “memory” is applied. Nevertheless, he does provide us with a way of understanding the world, and living creatures who inhabit it, that places the discussion in a different context from the usual one: the diverse features of human experience that we call memory are developed forms of basic patterns of relationship that characterize all actual entities. I will first review the relevant (...) features of Whitehead's conceptuality, then contrast the resulting view with its usual formulation, and then speculate about some forms of memory in Whiteheadian categories. This volume contains the proceedings of the Logic at Harvard conference in honor of W. Hugh Woodin's 60th birthday, held March 27–29, 2015, at Harvard University. It presents a collection of papers related to the work of Woodin, who has been one of the leading figures in set theory since the early 1980s. The topics cover many of the areas central to Woodin's work, including large cardinals, determinacy, descriptive set theory and the continuum problem, as well as connections between set (...) theory and Banach spaces, recursion theory, and philosophy, each reflecting a period of Woodin's career. Other topics covered are forcing axioms, inner model theory, the partition calculus, and the theory of ultrafilters. This volume should make a suitable introduction to Woodin's work and the concerns which motivate it. PURPOSE:To report the localization of a gene causing drusen and macular degeneration in a previously undescribed North American family. METHODS:Genetic mapping studies were performed using linkage analysis in a single family with drusen and atrophic macular degeneration. RESULTS:The clinical manifestations in this family ranged from fine macular drusen in asymptomatic middle-aged individuals to atrophic macular lesions in two children and two elderly patients. We mapped the gene to chromosome 6q14 between markers D6S2258 and D6S1644. CONCLUSIONS:In a family with autosomal dominant (...) drusen and atrophic macular degeneration, the gene maps to a 3.2-cM region on chromosome 6q14. This locus appears to be distinct from, but adjacent to, the loci for cone-rod dystrophy 7 (CORD7) and North Carolina macular dystrophy (MCDR1). Future identification of the gene responsible for the disease in this family will provide a better understanding of macular degeneration. (shrink)

Philosophers and scientists have maintained that causation, correlation, and "partial correlation" are essentially related. These views give rise to various rules of causal inference. This essay considers the "claims of several philosophers and social scientists for causal systems with dichotomous variables. In section 2 important commonalities and differences are explicated among four major conceptions of correlation. In section 3 it is argued that whether correlation can serve as a measure of A's causal influence on B depends upon the conception of (...) causation being used and upon certain background assumptions. In section 4 five major kinds of "partial correlation" are explicated, and some of the important relations are established among two conceptions of "partial correlation", the conception of "screening off", the conception of "partitioning", and the measures of causal influence which have been suggested by advocates of path analysis or structural equation methods. In section 5 it is argued that whether any of these five conceptions of "partial correlation" can serve as a measure of causal influence depends upon the conception of causation being used and upon certain background assumptions. The important conclusion is that each of the approaches (considered here) to causal inference for causal systems with dichotomous variables stands in need of important qualifications and revisions if they are to be justified. The philosophy of medicine cum bioethics has become the socially recognized source for moral and epistemic direction in health-care decision-making. Over the last three decades, this field has been accepted politically as an authorized source of guidance for policy and law. The field's political actors have included the National Commission for the Protection of Human Subjects of Biomedical and Behavioral Research, the Department of Health, Education, and Welfare, the President's Commission for the Study of Ethical Problems in Medicine and Biomedical (...) and Behavioral Research, the National Bioethics Advisory Commission, and the new Council on Bioethics; these groups and agencies have set forth rules on issues ranging from the role of humans in biomedical research to the production of human embryos for research, the definition of death, and the permissibility of human cloning. The members of the field are not just scholars and teachers in an academic realm directed to both theoretical and applied issues. They are, in addition, practitioners of a conceptual and moral trade that possesses a legal and political standing. This essay critically addresses the sudden emergence of bioethics as a societally recognized source of moral guidance, a source replete with authorized moral experts. Attention is directed to moral and conceptual assumptions that have led the philosophy of medicine, and especially bioethics, to acquire a quasi-juridical/political role in guiding clinical choices, framing health-care policy, and directing court holdings. The goal of this paper is to figure out whether Aristotle's response to the argument for fatalism in De Interpretatione 9 is a success. By "response" it is meant not simply the reasons Aristotle offers to highlight why fatalism does not accord with how we conduct our lives, but also the solution he devises to block the argument for fatalism. This paper finds that a) Aristotle's argument for fatalism is essentially bivalence plus that the truth of a proposition implies necessity, (...) b) that Aristotle's solution is to restrict bivalence, c) that this solution is coherent, and d) that while this solution does not rule out the possibility of fatalism, it does succeed in blocking the argument for fatalism offered within chapter 9. El objetivo de este trabajo es esclarecer si la respuesta de Aristóteles al argumento a favor del fatalismo en De Interpretatione 9 resulta exitosa. Por "respuesta" se entienden no solo las razones que da Aristóteles para destacar porqué el fatalismo no concuerda con la manera en que llevamos nuestras vidas, sino también la solución que ofrece para bloquear el argumento a favor del fatalismo. In 1966 the Brazilian physicist Klaus Tausk (b. 1927) circulated a preprint from the International Centre for Theoretical Physics in Trieste, Italy, criticizing Adriana Daneri, Angelo Loinger, and Giovanni Maria Prosperi`s theory of 1962 on the measurement problem in quantum mechanics. A heated controversy ensued between two opposing camps within the orthodox interpretation of quantum theory, represented by Leon Rosenfeld and Eugene P. Wigner. The controversy went well beyond the strictly scientific issues, however, reflecting philosophical and political commitments within the (...) context of the Cold War, the relationship between science in developed and Third World countries, the importance of social skills, and personal idiosyncrasies. Deleuze (1925–1995), in the early 1980s, adopts Peirce’s (1839–1914) semiotics in order to classify the signs that the images of the cinema display. Aiming at insufflating the Peircean principles with the movement that animates the images of cinema, he provides Peirce’s triadic logic with a new category – Zeroness – which stands for the semiotic movement of cinematic images. Deleuze’s new category has impacts on the main domains of Peirce’s philosophy. Accordingly, our inquiry will focus on the irradiation of Zeroness (...) over the (a) system of categories, the (b) Peircean phenomenology (phaneroscopy), the (c) generative categorial role, the (d) semiotic effectiveness, the (e) doctrine of continuity, and the (f) logic of relatives. This article will show that for Deleuze nearly like more recent Peircean scholarship: (a) zeroth state holds a categorial status, (b) some phenomenon instantiates zeroth state, (c) zeroth state plays a generative role regarding Peirce's categories, (d) zeroth state as a phenomenological category is semiotically effective, (e) zeroth state and the doctrine of continuity, and (f) zeroth state is a “medad.” In order to assess these topics, we recover Deleuze’s advances on Peirce’s philosophy and confront them with the Peircean studies on the zeroth state. Finally, we will see that Deleuze, far from being a Peircean scholar, developed the importance of the zeroth state for Peirce’s semiotics in advance to the subsequent Peircean scholarship. Reciprocally, most of Deleuzian scholars understimate the importance of Peirce for Deleuze’s semiotics. (shrink)

We show that it is consistent with ZFC (relative to large cardinals) that every infinite Boolean algebra B has an irredundant subset A such that 2 |A| = 2 |B| . This implies in particular that B has 2 |B| subalgebras. We also discuss some more general problems about subalgebras and free subsets of an algebra. The result on the number of subalgebras in a Boolean algebra solves a question of Monk from [6]. The paper is intended to be accessible (...) as far as possible to a general audience, in particular we have confined the more technical material to a "black box" at the end.
